From Thailand, studying in the UK. Presented with lethargy - Hb 81g/l with MCV 58.8 and RDW 31%. Blood film provided.

Anaemia and IgG paraprotein (50g/l). Trephine biopsy (H&E), trephine biopsy (Giemsa).

Female 14 months

Presented UTI. Noted to have hepatosplenomegaly and the following blood count parameters - Hb 45g/l WCC 16.5 x 10^9/l Platelets 246 x 10^9/l MCV 83. Father gives very similar family history but blood count parameters are now as follows - Hb 141 g/l WCC 12.0 x 10^9/l and Platelets 689 x 10^9/l. Female 75 years

Pancytopenia. No lymphadenopathy. Peripheral blood (slide 304510), trephine H&E, CD34 and Tdt (slide 304515).

Male 66 years

Epigastric pain and melaena. Hb 92g/l WCC 103 x 10^9/l Platelets 52 x 10^9/l. Peripheral blood (slide 304516), bone marrow aspirate (slide 304664).
