Female 54 years

History of abdominal pain and haematuria. Clinically considered to be renal colic. CT showed a lesion in the lower pole of the left kidney - ? perinephric abscess ? tumour. Past history of oophorectomy (1990). Laproscopic nephrectomy performed.



Specimen: Left kidney.

Macro: Specimen comprises of kidney weighing 210g and measuring 135mm (S-I) x 73mm (side to side) x 62mm (A-D). Staples are present in the hilar area. Cut sections show a pale well circumscribed but not encapsulated tumour involving the lower pole of the kidney measuring up to 45mm in maximal dimension. The tumour has a central area of haemorrhage, which appears slightly softened. The tumour appears to extend to the tip of the lower pole but does not appear to extend in to the adjacent perinephric fat. The adrenal gland is not present with the specimen.

Immunohistochemistry:
Positive for: Actin, desmin, HMB-45 and Melan-A.

Right renal mass.



Specimen: Partial nephrectomy.

Macro: Specimen received in two parts, including a 20x15x15mm nodule with a tan cut surface and a disc of renal parenchyma measuring 35x35x10mm which contained a round central defect 15mm in diameter.

Immunohistochemistry:
CD117 diffuse membranous positivity, CK7 and CD10 patchy positivity; RCC and vimentin negative.

Left laparoscopic radical nephrectomy for tumour.



Specimen: Kidney.

Macro: Left kidney 130x70x40mm with ureter 20mm and surrounding fat up to 70mm. In the superior aspect of the specimen, within the fat but separate from the kidney is a mass 90x70x60mm.

Immunohistochemistry:
WT1 positive. Vimentin, CD10, CK7 and CD117 negative.

Left kidney. Left simple nephrectomy with stone in renal pelvis with abscess. Prev psoas abscess.



Specimen: Kidney.

Macro: Kidney 16 x 9 x 6 cm. Slicing shows small kidney, atrophic, inflammation/necrosis and fat necrosis. No definite tumour seen. No stones noted.

Immunohistochemistry:
NONE; BUT SPECIAL STAINS e.g ZN, DPAS, GROCOTT'S - all negative.

Specimen: Partial nephrectomy.

Macro: Piece of renal tissue measuring 46 x 41 x 21 mm. On slicing, the tissue contains a partly cystic and partly solid tumour measuring up to 27 x 20 x 25 mm, which is well-circumscribed.

Immunohistochemistry:
CK 7 positive, high molecular weight cytokeratin positive, RCC marker negative, CD10 negative, AMACR negative.
