CD10, RCC, TFE3 positive. CAM5.2, CK7 and AE1/3 negative.

Left testicular tumour. AFP 11.1, beta HCG 124, LDH 309.



Specimen: Testis.

Macro: Testis 7.5 x 6 x 4 cm with 7.5cm x 3.4cm cord. The testis contains an ill defined pale tumour with areas of haemorrhage confined by the tunica.

Immunohistochemistry:
CD30 and OCT3/4 positive. Focal AFP and HCG positivity.

Female 58 years

Patient previously had biopsies of urethral tumour, found within a urethral diverticulum. Now anterior pelvic clearance.



Specimen: Transverse section (one representative half of megablock) of urethra from anterior pelvic clearance.

Macro: The urethra is dilated up to 30 mm diameter and filled by a tumour (arising within a urethral diverticulum). Focally the tumour erodes through the anterior vaginal wall.

Immunohistochemistry:
CK7 positive, PAX8 positive, CK20 stains very rare cells only. Ki67 stains 25% of nuclei. GATA3 negative, p63 negative.

Male 60 years

Right nephroureterectomy for ureteric tumour lower half of ureter with a right hydronephrosis and minimal cortex.



Specimen: Nephroureterectomy.

Macro: Right nephrectomy specimen with peri-renal fat: 210x130x90mm, and including 165mm of ureter. The mid 80mm of ureter appears thick-walled. The specimen in total weighs 738g. On opening, the kidney itself measures 105x60x20mm. The adrenal is not present. The kidney shows a hydronephrosis and marked cortical atrophy. The renal pelvis and proximal ureteric mucosa appear normal. A surgical clip is present approximately one third of the way down the ureter. On opening, the thickened ureter shows apparent luminal papillary tumour.

Thick, nodular and cartilaginous hydrocoels sac wall ?TB.



Specimen: Hydrocoele sac wall.

Macro: 2 pieces of firm pale tissue together measuring 64 x 55 by up to 9 mm. One surface is pale and nodular, the other surface is smooth, pale and slightly congested.

Immunohistochemistry:
Vimentin, pan cytokeratin and desmin positive with focal weak expression of CD99 and WT1 negative.

Right renal tumour, extending into inferior vena cava and involving paracaval lymph nodes.



Specimen: Radical nephrectomy.

Macro: Kidney showing tumour, 13.5cm diameter, involving renal vein.

Immunohistochemistry:
CD10 negative, CK7 very occasional cells positive, vimentin positive, AMACR positive.

Incidental retroperitoneal lymphadenopathy. Lymph node biopsy: mature teratoma.



Specimen: Right Radical Orchidectomy.

Macro: Testis, spermatic cord with coverings 45 x 27 x 25mm and cord 75mm. No abnormality on external examination. Slicing reveals a scar-like area 10 x 5mm towards lower pole. No other abnormality.

Specimen: TURP.

Macro: Prostatic chippings - multiple pieces of prostatic chippings together weighing 40g.

Immunohistochemistry:
Tumour shows strong positivity with P63 and shows positivity with MNF116 and 34Be12. The tumour cells are negative with chromogranin, PSA, CD56 and synaptophysin.

Presented with shortness of breath - on investigation R Pleural effusion and mass R kidney found.



Specimen: R kidney + hilar nodes.

Macro: Kidney 102 x 57 x 42mm.There is a white mass with nodular cut surface at the lower pole measuring 39 x 27mm. Hilar nodes appear grossly replaced by tumour.

Immunohistochemistry:
Positive for CD10, vimentin, P504S, E-cadherin, PAX8 Oct 3/4. Negative for 34BeE12, p63, CD117.

Testicular tumour.



Specimen: Left testis.

Macro: A radical orchidectomy specimen with the testis and epididymis measuring 60x40x25mm. The attached spermatic cord measures 75mm length. On sectioning the testis, there is a multifocal pale solid tumour measuring 25mm aggregate. There is an area of haemorrhage and necrosis within the central tumour. The tunica and cord appear free of tumour.
