Macro: Testis measuring 57x40x28mm. On sectioning, a tumour is seen at the upper pole.

Male 81 years

4cm lesion right kidney ?RCC.



Specimen: Right kidney.

Macro: Solid tumour 5 x 4 x 3.5cm in the inferior pole. Mahogony brown in appearance with a spongy microcystic appearance, appears to infiltrate into adjacent perinephric fat. No central scar. No venous invasion grossly.

Immunohistochemistry:
CK7.

Male 31 years

Haematuria and pain R flank. CT showed large R renal mass.



Specimen: Radical Right Nephrectomy.

Macro: Right kidney, Weight 1126g, measuring 220mm x 115mm x 90mm with upper pole tumour extending to the mid-zone measuring 120mm x 110mm x 75mm. The tumour shows haemorrhagic and yellow solid areas.

Immunohistochemistry:
Positive for CD10, P504S, E cadherin, TFE3 (nuclear) and cathepsin K. Negative for CK7, CK19, CK20, vimentin, CD117, CD15, HMB45 melan-A, AE1/3, 34 beta E12 or RCC antigen.

Specimen: Bladder chips.

Macro: Multiple fragments of cream/brown tissue 1.5 x 1.5 x 0.3cm in aggregate. All examined.

Chronic obstructive kidneys, abscess? Malignancy? Chronic infection.



Specimen: Left laparascopic nephrectomy.

Macro: Kidney received with perirenal fatty tissue in total 175 x 100 x 90 mm. The kidney itself measures 115 x 70 x 60 mm and shows dilatation of the calyces which are filled with pus. Cortex 3-12 mm thick. 40 mm of ureter are attached also containing pus. No tumour is identified.

Right renal tumour.



Specimen: Right radical nephrectomy.

Macro: The specimen (120x70x55mm) contains lobulated cream to yellow centrally placed tumour measuring 39x35x30mm. The tumour abuts the renal sinus and shows no haemorrhage or necrosis. Small areas of calcification are apparent. Renal vein is not involved. 90mm long ureter is included. Adrenal is not present.

Immunohistochemistry:
Pan CK, Synaptophysin and chromogranin uniformly positive. Ki67 expressed by up to 10% of tumour cells.

Multiple solid areas in the bladder. PMH of melanoma and pheochromocytoma.



Specimen: Bladder, biopsies.

Macro: Multiple tan coloured tissue pieces in aggregate measuring 20 x 15 x 4 mm.

Immunohistochemistry:
Positive S-100 and focal Melan-A. Negative chromogranin,synaptophysin, GATA-3, EMA, CK 20 and AE1/3.

Specimen: Left robotic partial nephrectomy.

Macro: Left partial nephrectomy weight 24g, measuring 50 x 37 x 22mm, containing an ovoid pale creamy brown, well circumscribed tumour up to 36mm in maximum dimension. Tumour reaches the resection margin (red ink).

Immunohistochemistry:
Cytokeratin AE1/AE3 positive, WT1 positive, CD57 positive, CK7 largely negative, CD34 negative, EMA negative, AMACR negative, RCC antigen negative, S100 negative.

Male 61 years

Left radical nephrectomy.



Specimen: Radical nephrectomy.

Macro: Total nephrectomy specimen measuring 110 x 60 x 100mm. No adrenal is attached. There is no tumour macroscopically in the renal vein. On opening the kidney there is a large well circumscribed tumour in the mid portion of the kidney infiltrating the cortex and medulla. The tumour measures 67 mm in largest dimension and shows focal areas of necrosis but otherwise dark brown/haemorrhagic cut surface.

Immunohistochemistry:
The tumour cells show strong diffuse positivity with HMB 45, Bcl-2. Ki67 shows only very occasional positivity. The tumour cells are negative with the following markers: MCK, S100, Inhibin, chromogranin A, CD31, calretinin, CD56, MNF116, synaptophysin, CD10, CD15, CK7, EMA and CEA.
