Male 49 years

History of left orchidectomy. Suspicious lump in right testis.



Specimen: Right testis.

Macro: Right testis measuring 65 x 40 x 45 mm with intact vaginal sac and 25 mm spermatic cord attached. Sectioning reveals a solid and cystic tumour measuring 40 x 30 x 40 mm confined within the tunica albuginea.

Immunohistochemistry:
GFAP and NSE highlighted glial tissue and neurons. OCT3/4 and Myogenin negative.

Female 73 years

Left open radical nephrectomy + hilar nodes. Appendicectomy. Had pelvic collection.



Specimen: Left open radical nephrectomy for renal mass.

Macro: Left kidney: A kidney measuring 20cm superior to inferior x15cm medial to lateral x 14cm anterior to posterior. There is a segment of ureter measuring 6x0.5cm. There is tumour measuring 15x11x9cm, involving most of the kidney, which appears to be confined to the kidney. Large areas are necrotic. There is no macroscopic involvement of the renal vein.

Immunohistochemistry:
tumour is diffusely positive for WT1, CD57 and PAX8 with scattered focal positivityfor CD56 (NCAM). EMA shows only extremely scanty focal positivity at one edge of the lesion but is negative elsewhere. CK7, racemase (AMACR), TTF-1 and CAIX are negative.

Male 31 years

End stage renal failure on dialysis. Acquired cystic kidney disease and had been followed up. On last scan a more solid lesion was seen.



Specimen: Left nephrectomy.

Macro: Atrophic and scarred kidney with multiple cysts, 5-35mm in size. Within the largest cyst was a solid lesion, 18mm, which is represented on submitted section.

Specimen: H&E is of tumour and adjacent uninvolved kidney.

Macro: 3cm well circumscribed tumour with tan, variegated cut surface.

Immunohistochemistry:
Pan-cytokeratin (AE1/3) focally positive, Vimentin positive, CD10 focally positive, HMB45 negative, TFE3 (done elsewhere) positive. FISH analysis (done elsewhere): TFE3 gene rearrangement.
