Left pyonephrosis 10 weeks. Unwell, weight loss.

Specimen: Radical Nephrectomy.

Macro: A kidney with attached perinephric tissue weighing 274g. The kidney measures 74x50x35mm with attached perinephric fat up to 30 mm. Adrenal gland is not identified. The ureter appears to be cut almost flush to the periureteric junction. On bisection, renal pelvis is dilated to 52mm maximum dimension. Renal calyces and adjacent cortex show multiple soft yellowish areas suggestive of suppuration measuring up to 18mm maximum dimension throughout. Renal pelvis is also markedly congested. Renal cortex is approximately 4mm thick.

Immunohistochemistry:
CD68 positive,pancytokeratin negative, EMA negative

Left testicular pain treated with antibiotic but was useless. USS suggest left testicular Ca.

Specimen: Left testis.

Macro: Left Testicle: The testis measures 60x46x30mm. The spermatic cord measures 75x15mm. On sectioning the parenchyma is entirely replaced by white with verigated colour tumour measuring 40x35x28mm.The tumour abuts the the tunica albuginia but does not appear to breach it. Macroscopically the tumour appears confined to the testis. The spermatic cord appears unremarkable. On further sectioning the tumour shows focal haemorrhage with possible areas of necrosis.

Immunohistochemistry:
None
