Renal u/s performed for microscopic haematuria showed a 2.3cm lesion in the interpolar region of the left kidney. On CT scan shown to be solid and enhancing. Nephrectomy performed.

Immunohistochemistry:
Positive - CK7 EMA CD57 racemase. Negative - WT1. Ki67- low proliferation rate.



Specimen: Nephrectomy Specimen.

Macro: Well circumscribed creamy white tumour 26mm in max dimension. Confined to the kidney. Background renal tissue appeared normal.

Right ureteric tumour. Nephro-ureterectomy done.



Specimen: Ureter.

Macro: A fragmented kidney with 160mm length of ureter. The ureter shows a nodular tumour in the lumen and infiltrating the wall measuring 8x6mm situated 80mm from the distal end.

Positive with ASMA, Desmin & CD34. Negative with S100, AE1/AE3, CK7, CK20, 34beta E12, PSA, CK5/6, Calretanin, BEREP4. Further IHC - positive for PR, neg for C kit, ALK1, Ki67 low.



Specimen: TURP.

Macro: 58grams of chippings.

CT scan shows a right renal mass.

Immunohistochemistry:
Diffuse strong positivity for AE1/AE3, Racemace, CK7 and E-Cadherin. Patchy positivity for CD15, and negative for CD10 and RCC.



Specimen: Right radical nephrectomy.

Macro: Right kidney 150x75x60mm with ureter measuring. The upper pole has a spherical tumour 65mm confined to the kidney. The pelvi-calyceal system renal vessels at hilum are unremarkable.

Presented with left sided loin pain and haematuria. Recurrent UTIs. Imaging showed a left renal mass. Also history of sickle cell trait.

Immunohistochemistry:
AE1/3, CK903, Cam5.2 EMA, CK7 positive, Vimentin variably positive, CK20 focal positivity. CD10, CEA, TTF-1, Desmin, OCT3/4 PLAP, PAS negative.



Specimen: Left kidney.

Macro: Tumour mass in the upper pole of the left kidney measuring 5x4.5x3.9cm. It had a pale grey cut surface. In addition, there were multiple separate tumour nodules in the renal parenchyma, renal hilar fat and perinephric fat. Adrenal gland contained tumour deposits. The renal pelvis contained tumour ?TCC.

Male 37 years

History of melanoma removed from right shoulder 2 years previously. Presented recently with haematuria. Cytoscopy showed 3cm melanotic solid tumour on posterior and right lateral bladder wall. TURBT and cold cup biopsy of tumour base.

Immunohistochemistry:
Strongly positive for Melan-A, NK1C3, S100 protein and HMB45 (patchy). Labelling for high mol. Wt cytokeratin (34BE12), CK7 and CK20 is negative.



Specimen: Bladder tumour, TURBT specimen.

Macro: Multiple pieces of grey brown tissue weighing 5grams.

Male 59 years

History of large 'abdominal' mass detected on routine clinical examination. Biopsy suggestive of renal cell carcinoma. Subsequent left radical nephrectomy with part of colon, pancreas and spleen removed.

Immunohistochemistry:
CD117, CD10 and CK7 positive. Variably positive for AE1/3. Focal vimentin positive. Negative staining for Melan A, Inhibin, CK20 and chromogranin.



Specimen: Left nephrectomy.

Macro: Left kidney with tumour measuring 16x17.5x14cm. The tumour has a nodular grey homogenous cut surface with small areas of haemorrhage. The tumour has largely replaced the kidney. The renal sinus contains tumour. The mass was also adherent to part of the colon and pancreas removed in one area.

Right nephrectomy for tumour.



Specimen: Right nephrectomy.

Macro: Right kidney and perinephric fat weighing 460g. Kidney measures 110mm pole to pole. In the upper mid zone on the anterior surface there is a tumour nodule 3.2mm in diameter. This has a haemorrhagic microcystic appearance.
