Right testicular tumour with lung metastases.

Immunohistochemistry:
Most of the tumour is positive for hCG and negative for CD30, PLAP and AFP. A solitary tiny focus of tumour is positive for CD30 and PLAP and negative for hCG and AFP. PLAP also stains the ITGCN.



Specimen: Right testis and spermatic cord.

Macro: An expanded 50mm diameter testis with 60mm spermatic cord. The visceral tunica vaginalis is smooth and intact with no evidence of tumour involvement. On sectioning most of the testicular volume is seen to be occupied by a well circumscribed, haemorrhagic mass 43mm in diameter. The tumour is confined to the testis.

Right Renal Tumour.



Specimen: Right Nephrectomy.

Macro: Right kidney covered in fat 220x120x110 mm with attached ureter 100 mm in length. The kidney measures 120x60x60 mm. A well circumscribed brownish tumour measuring 35mm in diameter is present in the mid-pole showing a central scar. The pelvicalyceal system, renal vessels at hilum are unremarkable.

Specimen: Testis.

Macro: A. A 10 mm soft tissue fragment specimen submitted for frozen section. B. Testis 40 x 30 x 18 mm containing a well circumscribed tan coloured nodule 30 x 22 x 17 mm. Specimen is tissue adjacent to nodule bed.

Immunohistochemistry:
Positive: Inhibin, S-100, Calretinin, Melan-A. Negative: PLAP.

Scrotal tumour, distinct from testicle and epidermis ? malignant.



Specimen: Mass right scrotum.

Macro: A well circumscribed mass measuring approximately 43 x 40 x 35 mm with a variegated cut surface with glistening rather gelatinous white areas and yellow areas. No testis is present in the specimen.

Immunohistochemistry:
Vimentin positive, S100, neurofilament, desmin, actin, cytokeratin, CD34, chromogranin, c-kit/CD117 and inhibin negative. Oestrogen receptor is positive in 50% of cells.

Immunohistochemistry:
Cont... PR negative. Ki-67 staining reveals a low proliferation rate of <1%.

Right testicular tumour.



Specimen: Right orchidectomy.

Macro: Testes and coverings 67 x 35 x 36 mm with cord 60 mm in length. The testes measures 45 x 35 x 37 mm, slicing reveals a solid yellow firm tumour 21 x 22 x 24 mm with focal areas of haemorrhage.

Immunohistochemistry:
Negative PLAP, CD117, CD30, Pan CK, CD20, CD3, alpha-fetoprotein and HCG. Focal weak positive with LCA and CD138.

Specimen: Left nephrectomy.

Macro: Dark brown tumour in lower pole, 60 x 60 mm. Extensive necrosis and haemorrhage. Focally infiltrates perinephric fat.

Immunohistochemistry:
Strong membranous staining for CK7. Focal staining with CD10. Negative for vimentin and CD117.
