TURP for urine retention.



Specimen: TURP Chippings.

Macro: Prostate chips. Multiple fragments of prostate chippings measuring 8.0 x 6.0 x 2.0 cm and weighing 22 grams. All the tissue was embedded.

Immunohistochemistry:
Immunostaining shows that these lymphocytes stain positively and diffusely using CD20 and bcl-2, and patchily in about 50% of the cells using CD43. They are negative with CD3,CD5,CD23,bcl-6, CD10 and cyclin D1.

History of left partial epididymectomy for painful epididymis with incidental finding of a nodule diagnosed as seminoma (13 mm). Now completion left orchidectomy.



Specimen: Testis.

Macro: Orchidectomy specimen including 5 cm of cord, testis 3.2 x 3 x 2.5 cm. There is an irregular yellowish white fibrotic area in the hilum (rete testis) containing suture material measuring 2 x 1.5 x 1.5 cm. Vague nodularity is revealed on further slicing. The remaining epididymis and testis appear unremarkable.

Incidental bilateral renal tumour. Excision of left 6 cm lower pole tumour.



Specimen: Partial nephrectomy.

Macro: Partial nephrectomy with a tumour 60 x 50 x 40 mm, partially attached to perinephric fat. The tumour is yellowish-brown with areas of haemorrhagic necrosis.

Raised haemorrhagic lesion. Previous biopsy - foreign body giant cell reaction.



Specimen: Bladder biopsies.

Macro: Irregular fragments of tissue up to 8 mm in diameter.

Immunohistochemistry:
Immuno staining of the amyloid deposits was performed using monospecific antibodies reactive with serum amyloid A protein (SAA), apoliprotein A1 (apoA1) and with Kappa and Lambda immunoglobulin light chains.

Macro: Testis 55 x 35 x 25 mm with attached spermatic cord 60 mm long. Slicing reveals a well circumscribed tan tumour 9 mm in diameter confined to the testis.

Male 28 years

About four years before had cystic/solid mass removed from epididymis. Now complaining of painful atrophic testis.



Specimen: Orchidectomy.

Macro: Thickened paratesticular tissue up to 4 mm, around much of the testis, it is probably separate from the epididymis. The testis itself appears normal.

Immunohistochemistry:
Positive for cytokeratins (broad spectrum), Ck7, CK 5/6, calretrinin, WT1, BerEP4, Ca 19.9, Ca 125, CD10, oestrogen receptor. Ki67 less than 5%. Negative for CK20, HMBE1, CDX2, S100, renal cell cancer marker, vimentin.
