Complex cyst lower pole of left kidney. ?Malignant ?margins.



Specimen: Partial nephrectomy.

Macro: The specimen consists of the lower pole of kidney measuring 60x45x35mm, bearing a mass measuring 50x25x35mm. On cut section, two lesions are grossly identified - 1 superficial & soft haemorrhagic mass measuring 50x 35x30mm & a further deeper & solid partly necrotic mass, measuring 35x20x25mm. Both surfaces inked green. Grossly, the renal resection margin is at least 5mm away from the deep necrotic mass as described.

Immunohistochemistry:
CD10+ Mib1 - 70% CD19+ bck-2+ CD5 o53beg CD20+ CD79+ bcl-+/- CD23- MUM-1+FOX-P1+.



Specimen: Right testis.

Macro: Testis and cord weighs 173gm. There is a large cream coloured mass within the testis which measures up to 6 x 4 x 4.5cm. This covers most of the testis and is seen extending up the spermatic cord.

Macro: Specimen consists of testis & appendages weigh 36gm. There is attached spermatic cord up to 7.5cm with testis 5x3 x2cm. On sectioning the testis there is a tumour which measures up to a maximum dimension of 1.2cm imdway down the testis to one edge. It is seen in up to 4 slices, giving a maximum thickness of about 1.3cm.

Had orchidectomy following presentation with a testicular swelling.

Immunohistochemistry:
Tumour cells diffusely positive with alpha-inhibin and vimentin, and scattered cells positive with Melan A.



Specimen: Orchidectomy.

Macro: Cut section of the testis showed a well circumsribed nodule with a uniform tan-coloured cut surface, abutting but not invading the tunica albuginea. The rest of the testis, the epididymis and spermatic cord were normal.

Specimen: Horse shoe kidney.

Macro: On gross examination the kidney showed a 60 x 50 mm soft yellowish brown variegated tumour with focal haemorrhage infiltrating the perinephric and sinus fat. The retro-caval and para-caval lymph nodes contained metastasis.
