Specimen: Right partial nephrectomy.

Macro: Partial nephrectomy measuring 38x30x15mm bearing a nodule measuring 33x20x24mm, lying 5mm from the deep surgical margin and demonstrating a homogeneous pale grey cut surface.

Immunohistochemistry:
HMB45 patchy positive, Melan A focally positive, Desmin peripheral positivity.

Specimen: Orchidectomy.

Macro: This is a testis measuring 56 x 36 x 24mm with attached spermatic cord measuring 57 x 12 x 14mm. The tunica vaginalis is mostly intact with a small defect inferiorly. On slicing within the lower half of the testicle there is a circumscribed tumour measuring 15 x 19mm. It is composed of pale and yellow tissue with focal haemorrhage. It appears confined to the testis and does not involve the tunica albuginea. The rest of the testis and the epididymis are unremarkable. The spermatic cord appears normal.

Immunohistochemistry:
Positive for OCT3/4, AE1/AE3 and CD30. Negative for PLAP, Glypican 3 and AFP.

Male 76 years

Long term indwelling catheter with frank haematuria. Diagnostic cystoscopy showed a right post wall exophytic mass and sent for TURBT. No pelvic mass, clear CT urogram.

Specimen: Left radical nephrectomy.

Macro: Two tumours - one in the upper pole and one in the lower pole. Both are solid brown roughly spherical tumours with foci of haemorrhage. Slide submitted is from lower pole tumour.

Immunohistochemistry:
Similar immunoprofile in both tumours: CK7 and CD117 neg. Ecadherin patchy positivity. EMA positive. CD10 focal membrane positivity. Racemase shows weak positivity with occasional darkly stained cytoplasmic inclusions. Vimentin negative in the majority of tumour cells. Pancytokeratin MNF, CK20, RCC, pax8, BerEP4, HMB45, MelanA are all negative. Immunohistochemistry for SDHB is negative . Fumarate hydratase expression retained.

Left testicular tumour.



Specimen: Left testis (left radical orchidectomy).

Macro: Testis, spermatic cord and coverings, together 148g. The spermatic cord measures 50mm in length. The testis itself measures 70x55x50mm. The bulk of the testis is replaced by a tumour mass 65mm in maximum dimension that has a solid homogenous grey cut surface with focal haemorrhagic areas. The tumour appears confined to the testis and does not invade the tunica vaginalis, epididymis or hilum.

Immunohistochemistry:
Positive for CD20, CD79a, MUM1, BCL6 Negative for CD3, CD5, CD10, CD21, CD23, CAM5.2, Oct 3/4, AFP, CD117. Ki67 is high (80-90%).

Specimen: Right kidney and ureter.

Macro: Kidney with peri nephric fat the whole specimen measuring 14x9x5cms. There is no adrenal gland identified. Attached is 6cm of proximal ureter. On slicing the kidney in the mid portion there is distension of the urinary collecting system by a fleshy lesion measuring 1.2x1cm. There is nothing visible within the renal pelvis or ureter.
