Male 25 years

Left testis 1cm cystic area with mural thickening. Tumour markers normal. ?Testicular tumour ?Leydig cell tumour.



Specimen: Left Orchidectomy.

Macro: Received testis (70x40x40mm) with attached cord (90mm in length). On sectioning, there is a cystic lesion 18mm in maximum dimension with some pale nodular lining. The cyst is well demarcated from the normal looking surrounding parenchyma.

Immunohistochemistry:
Positive: MOC31, CK7, CD15, CA125, PLAP, ER, PR, WT1 Negative: CK20, Calretinin, CEA.

Specimen: Cystoprostatectomy.

Macro: Received is a radical cystoprostectomy specimen. Bladder including perivesicle fat is 230 x 140 x 50mm. On slicing there is ill defined tumour and some adjacent cystic areas towards the mid right wall up to 20mm. Section from cystic area.

Scrotal mass, ultrasound suggests tumour.



Specimen: Orchidectomy.

Macro: Orchidectomy specimen including testis measuring 37 x 15 x 30mm with a mass measuring 95 x 75 x 75mm present at the lower pole of the specimen. The mass is fairly well-circumscribed and solid with a light brown and white cut surface and a central necrotic area. The mass appears to be separate from the testis and is present within soft tissue.

Immunohistochemistry:
FISH shows MDM2 amplification.

Right upper pole renal tumour.



Specimen: Partial nephrectomy.

Macro: A partial nephrectomy specimen measuring 50x25x20mm. The surgical resection margin show has been painted green. Cut surface reveals a circumscribed yellow tan lesion approximately measuring 20x22x20mm and has an approximate clearance of 2mm from the surgical margin.

Immunohistochemistry:
Positive for CA9, vimentin, CK7, CD10, AE1/AE3, EMA and negative for P504s, RCC, CD117, TFE3, CK20.

Left laproscopic nephrectomy.



Specimen: Nephrectomy.

Macro: Kidney - radical nephrectomy specimen measuring 180 mm x 80 mm x 50 mm. The kidney itself measures 100 x 50 x 40 mm. In the upper pole there is a simple cyst measuring 60 mm in maximum dimension and adjacent to this there is a mahogany brown tumour with a central scar measuring 60 x 40 x 60 mm. Tumour with possible pushing into a vessel noted and sampled in block provided and macro photograph.

Immunohistochemistry:
CK7 and CD31 provided. CD117 positive, negative for vimentin.
