Female 25 years

Left radical nephrectomy for renal tumour.



Specimen: Radical nephrectomy.

Macro: A radical nephrectomy containing a tumour in the kidney measuring 115 mm in maximum dimension, with a pale rubbery cut surface and a lobulated appearance with yellow, gelatinous and haemorrhagic areas. It is located mid-kidney and in the lower pole.

Immunohistochemistry:
Epithelioid areas with tubules and sheeted compact (non-spindle cell) areas diffusely positive for PAX8, AE1/3, WT1 and CD56. MIB-1 >70%. Negative for chromogranin, synaptophysin, MIC-2 (CD99), desmin and myogenin.

Male 44 years

Right testicular lump Elevated AFP, LDH, HCG.



Specimen: Right orchidectomy.

Macro: Right testis measuring 80 x 45 x 50mm. Spermatic cord 30mm length. Weight 115g. The testis contains a 50 x 40 x 40mm firm grey tumour, which has a partly lobulated and cystic appearance. Tumour appears clear of the epididymis. There is a fine rim of normal testicular tissue surrounding the tumour, and the rete testis is difficult to identify.

Immunohistochemistry:
Provided. AFP, HCG, MNF116, CD30 & OCT3/4.

Male 31 years

Left radical inguinal orchidectomy for testicular cancer.



Specimen: Left testicle.

Macro: A testis measuring 75x45x35mm with attached spermatic cord measuring 40mm in length and up to 7mm in diameter. The external surface of the specimen appears smooth and intact. On slicing, almost the entire testicular parenchyma is replaced by a multinodular solid yellow-white tumour measuring 60x42x35mm. Macroscopically the tumour does not involve the epididymis or the tunica vaginalis.

Lesion in dome of bladder.



Specimen: Partial cystectomy.

Macro: Polypoid lesion measuring 25x23x15mm with underlying bladder and fat measuring 100x60x15mm. There is a thin cuff of bladder mucosa around the lesion.

Female 73 years

Non-functioning kidney.



Specimen: Left nephrectomy.

Macro: Kidney with attached perinephric fat. The whole specimen measures 170mm from pole to pole. Vascular clips are present at the presumed hilum. On slicing the kidney measures 135mm from pole to pole. There is extensive dilatation of the whole of the pelvicalyceal system and the calyces are filled with yellow/green pus. There is severe cortical atrophy associated with extensive fibrosis and areas of necrosis. Some of the necrotic areas extend into perinephric fat. No mass lesions are identified.

Right partial nephrectomy.



Specimen: Right partial nephrectomy.

Macro: Partial nephrectomy with a well circumscribed tan brown lesion, 24mm in size, abutting the capsule and clear of margins.

Immunohistochemistry:
Negative: CK7, Melan A, CAIX, MNF116, Pax8, HMB45, CD117 (stains numerous mast cells also Toluidine blue positive), Stat6, EMA, Bcl2 (some background cytoplasmic staining but no nuclear positivity in tumour cells). CD99 focally positive. Provided: Macro image, CD34, SMA and electron microscopy of the tumour.
