Left renal mass. Left partial nephrectomy.



Specimen: Left partial nephrectomy.

Macro: Wedge of kidney measures 30 x 30mm across the margin and up to 25mm thick with attached peri-nephric fat up to 45mm in length. The specimen has been incised prior to receipt which may affect assessment of margins. On slicing there is a well-circumscribed nodule which is partially exophytic measuring 20 x 18 x 20mm. The margin appears to be 2mm. The margin appears to be 2mm.

Male 40 years

Presented with testicular torsion 2 weeks ago. Declined treatment initially but returned with ongoing symptoms.



Specimen: Orchidectomy.

Macro: Testis measuring 47x39x37mm with spermatic cord 35x20mm and attached soft tissue. The entire specimen is extensively haemorrhagic.

Male 64 years

Right lower pole renal tumour.



Specimen: Right nephrectomy.

Macro: A right kidney with attached perinephric fat 13.5 x 7.5 x 6cm and 5cm of normal ureter, weighing 410g. The adrenal is not present. The kidney is 12.5 x 6.2 x 4.8cm. Occupying the lower pole of the kidney is a firm solid yellow tumour 4.5 x 4.3 x 4.8cm. There is a white glistening myxoid area and probable areas of necrosis. The tumour appears to push and expand into the renal capsule, but it does not obviously invade across it. Anteriorly the tumour has a slightly irregular outline and bulges of tumour appear to extend into the perinephric fat, but they may be confined within an attenuated renal capsule. At the lower pole of the tumour there are three plastic and two metal clips on the outer surface of the perinephric fat. There are possible tongues of slightly yellow/beige tumour within distended renal vein tributaries superior to the tumour within the kidney. No hilar nodes identified.

Specimen: Robotic assist lap prostatectomy.

Macro: A prostatectomy 5cm transverse. 4.4cm apex to base. 3.9cm anterior to posterior with attached seminal vesicles, weighing 68.3g. Specimen painted right lobe green, left lobe orange. Apex, base and anterior strip black.

Small renal mass, ?RCC.



Specimen: Right partial nephrectomy.

Macro: A partial nephrectomy specimen measuring 32 x 22 x 20mm depth. There is a well circumscribed lesion measuring 21mm maximum dimension, with a solid, pale cut surface. The deep margin is 2mm clear.

Immunohistochemistry:
CK MNF116 and WT1 strongly positive. S100, EMA and Desmin negative.

Specimen: TURBT.

Macro: A mass of blood clot - 30x30x10mm - embedded in 3 cassettes.

Immunohistochemistry:
CD68 highlights giant cells. Focal positivity for SMA. Negative for cytokeratins (AE1/3, MNF116, CAM5.2, 34BE12, p63, CK7, CK20). Negative for CD34, S100, CD31. PSA negative.

Found hard lump in the right testis a few weeks ago Some discomfort and pain in the right testis Ultrasound showed ? testicular tumour.



Specimen: Testis.

Macro: Testis weighing 48 g with attached cord. The testis measures 65 x 40 x 28 mm, and the cord is 40 x 22 mm. The testicular coverings are intact. On slicing there is a well circumscribed yellowish/brown lesion measuring 20 x 20 x 12 mm, confined to testis.

Immunohistochemistry:
Staining for Inhibin and Vimentin; negative staining for CK7 and PLAP.

Macro: Kidney replaced by tumour - 180x110x130mm. large cysts with smaller solid excrescences with grape-like architecture. Solid nodule maximum 90mm in diameter with bubble wrap-type cut surface.

Immunohistochemistry:
Positive for MNF116, AE1/3, vimentin, ecadherin, PAX8, Racemase, SDHB. Negative for 34BE12, CD10, CK7, CEA, CAIX, FH.

Positive (diffuse) PAX8, EMA, CK8/18, vimentin, AMACR Patchy positive : Pan CK (AE1/3) and more focally for CK7 ALK1 cytoplasmic positivity but no nuclear positivity. Negative for HMWCK, p63, CK20, CD10, CD117, CAIX, melan a, hmb45, SMA, desmin, S100, CD31, CD34, GATA3, WT1, Caldesmon, DOG1, OCT3/4, inhibin, CD99, HCG, Glypican 3, TFE3, No evidence of SDH or FH loss.


AC20 - Educational Case

Female 43 years

Left partial nephrectomy.



Specimen: Partial Nephrectomy.

Macro: 23x30x19mm well defined tumour.

Immunohistochemistry:
Positive for CD117, PAX8, pancytokeratin, Cathepsin K. Focally positive for CD10. CK7 shows scattered positive cells staining. No loss of FH or SDHB Negative for EMA, AMACR, Vimentin, CA9,HMB45, melan A, TFE3, MOC31, CK20, ALK-1. Molecular: No evidence of chromosome loss or gain for chromosomes 1, 2, 6,10 and 17 in the 100 cells examined with each probe set. No evidence of a TFEB rearrangement in 100 cells examined.
