Male 71 years

History of large painless swelling left groin. ?Lymph node. Tumour is positive for MNF116 and CAM 5.2, CK7 and CK20 are negative. Prostatic specific antigen and prostatic acid phosphatase are positive.



Specimen: Left groin lymph node.

Macro: Ovoid mass measuring 4 x 3 x 2.5cm with a firm and pale cut surface.

Renal tumour. Immunohistochemistry shows positivity for cytokeratin AE1-3, EMA, CD10 and CD117. Negative for CD15 and Vimentin.

Macro: An upper pole tumour 17cm in maximum dimension. Cut surface is tan-yellow with a central fibrotic area.

Cystic tumour upper pole of right kidney - partial nephrectomy. Tumour ruptured. Total nephrectomy. No immunohistochemistry performed.



Specimen: Kidney.

Macro: Upper pole of kidney, 60 x 60 x 30mm largely replaced by tumour which is disrupted making overall measurement difficult. Tumour appears confined by renal capsule.

Right scrotal mass. ?Epididymal tumour. No trauma. No urinary symptoms.



Specimen: Right testis.

Macro: Right testis, 11 x 7 x 5cm with white paratesticular mass 6 x 6 x 2.5cm. The tumour is lobulated and grey with haemorrhagic areas. Testis macroscopically normal.

Pedunculated bladder tumour with 1 sm satellite. Foot of tumour seemed v. firm. ?Invasive. Not completely resected. Immuno's - Epithelial elements - positive for pan-cytokeratin, CK5/6, CK19, CK20, 34BE12, CAM 5.2. Giant cell and mononuclear elements - negative with above epithelial markers, do not express BHCG. Strongly positive with Vimentin, CD68 and tartrate resistant acid phos.
