Skin, excision biopsy. Very vascular tumour right thigh 2-3months ?Angiokeratoma. ?MM.

Macroscopic description:
an ellipse of skin measuring 22x10x up to 9mm; bearing a 7x6x2mm centrally raised grey papule with an indurated surface. The lesion extends to 2mm from the nearest margin. The lesion is submitted in blocks A&B (not all embedded).

Immunohistochemistry:
The lesional cells are negative for CD34, CD31, melan-A, actin, desmin and S100.

Incisional biopsy right cheek. Longstanding lesion right face. Growing in size recent past (over few weeks). Spreading through right face.

Macroscopic description:
A skin ellipse 9x3mm to a depth of 9mm. Skin surface unremarkable.

Immunohistochemistry:
CD20, Bcl 2, Bcl 6 and MUM1 strongly and diffusely positive. Ki67 brisk. Occasional cells positive with Cyclin D1 and CD30. Negative for CD3, CD5, CD21, CD23 and EBER-ISH.

Incisional biopsy. History of crusted blistering lesions. New lesion chest now. Very ulcerated mouth and genitals.

Macroscopic description:
Tan biopsy of the skin 4x4x5mm.

Immunohistochemistry:
Immunoflourescence showed strong intercellular IgG and moderate intercellular C3 in the epidermis. IgM and IgA were both negative.

Macroscopic description:
Skin ellipse 40x29x8mm. A friable tan warty nodule 28mm.

Immunohistochemistry:
Positive - CK8/18, BerEP4, CD56. Focal positive: EMA. Rare cells positive: 34βE12. Negative: CK7, CK20, TTF-1, chromogranin A, synaptophysin, CEA, p63, CK5/6. ABPASD: no mucin.

6mm punch biopsy - back. Blistering rash with epidermal detachment - TEN ? Drug ? Infection. Started off looking like an exanthem.

Immunohistochemistry:
IgG, IgG4, IgA, IgM, C3 and fibrin are negative.

Punch biopsy: 4mm diameter and 6mm depth. Papillomatous areas along one edge.

Immunohistochemistry:
CK7, CK20, p63 - positive. CDX2 moderately positive. ER, MelanA, S100 - negative.

Macroscopic description:
Specimen pot labelled - biopsy scalp. Received is a skin lesion measuring 10x5x5mm. There is a central keratotic nodule measuring 3mm in maximum dimension x 2mm in height. No tissue remains.
