Macroscopic description:
An ellipse of skin measuring 20x10x8mm. Bisected longitudinally.

Immunohistochemistry:
The tumour cells express CD4, CD7, CD30, Perforin and ALK1 (nuclear and cytoplasmic). They are negative for CD2, CD3, CD5, CD8, CD56 and CD57. Staining for Epstein-Barr virus (LMP1, EBER) is negative. CT Chest/abdomen/pelvis revealed generalised lymphadenopathy.

Episodes of erosive/blister like lesions on trunk, neck, legs, hands. Five month history. Transient lesions which last about 1 month, come and go leaving a slight scar. ?Sweet's syndrome. Differential diagnosis erythema multiforme.

Immunohistochemistry:
epidermal lymphocytes positive for CD3, CD8, CD56. They are negative for CD5 and BF1. Scattered CD30 positive cells in dermis

Differential diagnosis: ?Panniculitis. Sent for culture. ?Sarcoidosis. Three week history of multiple pink/purple nodular subcutaneous lesions appearing on arms and legs, approximately six to eight in number and measuring 0.5cm to 1cm in maximum diameter. Some showing resolution after developing a punctum and discharging white pus like exudates. Some static, although now dusky purple colour and have become fluctulant as they were at onset. Past medical history: Known glioblastoma - surgery feb 2012. Temozolomide and radiotherapy in April and May 2012. Persistent extention of T.E. Cranial bone flap infection. Peroneal ulcer and paracolic abscess drained. Cultured - VRE anaerobes and yeast. Drugs: Tinzaparin, Sodium Valproate, Paracetamol, Vancomysin, Clobazam, Dexomethasone, Lamotrigine and Omeprazol.
