Specimen: Skin dorsum left foot.

Macroscopic description:
Suture orientated skin ellipse measuring 18 x 9 mm with up to 5 mm subcutaneous tissue. There is a grey-brown dome-shaped lesion on the surface measuring 8 mm maximum. Slicing reveals a nodular lesion which is dark brown at the base and measures 7 mm across and up to 6 mm in thickness.

30 year history of hyperkeratotic plaques on the scalp and face ? Darier's disease.

Macroscopic description:
Punch biopsy of skin 4 mm in diameter x 4 mm deep.

Other investigations: HIV positive. TB not proved on lymph node culture. T-spot test positive.
