Macroscopic description: A piece of cream tissue measuring 4 x 3.5 x 2 mm. Bisected and all embedded. Immunohistochemistry: None performed.

Multiple lichenified plaques on the trunk and limbs.

Specimen: Incisional biopsy.

Macroscopic description: Ellipse of skin: 15 x 5 x 4mm. Darkly pigmented and roughened. Immunohistochemistry: PAS (+), Congo red (weakly +), Sirius red (-), Thioflavin T (strong immunofluorescence), AE1/AE3 (weakly +).

Right buttock excision of lesion, ? MM.

Specimen: Right buttock.

Macroscopic description: Ellipse of pale tan skin measuring 25mm x 17mm with underlying fibrofatty tissue to a depth of 16mm. The skin surface bears a well-defined raised firm tan and white lesion which measures 15mm x 11mm x 5mm and is 2mm from the nearest peripheral margin. The excision margin is inked blue and the specimen is serially sliced through the lesion. Slicing reveals that the lesion has a firm pale tan cut surface and extends to up to 9mm deep. All lesion taken. A1-A3 - 1:1. +GS. HJ/JN. Immunohistochemistry: N/A.

Says can feel a lump again at the site of recent surgery. Feels tender at times. Catches when shaves his scalp. Examination scalp/nape of neck posteriorly left side small lump underneath scar. Non tender excised. ? Small epidermoid cyst or only fibrosis.

Macroscopic description: Irregular piece of skin 15 x 10mm with underlying tissue up to 8mm. On slicing no obvious cyst seen. Serially sliced. Immunohistochemistry: n/a.

Pigmented nodular lesion on lower outer helix of left ear.

Specimen: Excision biopsy - lower outer helix of left ear.

Macroscopic description: Elliptical piece of skin with subcutaneous tissue measuring 21x10x3 mm. Skin surface shows an elevated variably pigmented lesion with irregular borders measuring 6x5 mm. Immunohistochemistry: not performed.

Specimen: Skin biopsy left foot (immuno's sent).

Macroscopic description: Skin ellipse. A 10 mm papillomatous lesion arises from the skin surface. Transverse sections taken through the lesion. Immunohistochemistry: SOX 10, S100,GFAP, MNF116 immuno's scanned and available. CD34 Negative.

A 4mm darkly pigmented lesion right upper arm. Starburst appearance on dermoscopy. ?Spitz naevus, rule out melanoma.

Macroscopic description: A skin ellipse 15x7x5mm with a central pigmented macule measuring 3x3mm. Immunohistochemistry: None.

Macroscopic description: Skin ellipse 13 x 12 x 3mm deep. There is a yellow nodule measuring 7 x 6mm which is 2mm from the nearest resection margin. Immunohistochemistry: ki67.

Left wrist, Right abdomen, PRAME negative, RNA fusion panel: BRAF::AGK fusion is detected. NGS M119.5 panel: No driver variants detected. No TERT promoter variant. Copy number changes - Segmental 7 q gain.

Right abdomen. PRAME negative, RNA fusion panel - AKAP9::BRAF fusion was detected. No oncogenic variants were detected in BRAF, NRAS or KIT. Copy number variation summary: AMPLIFICATION: None GAIN: segmental 7p, segmental 7q, segmental 20q LOSS: segmental 7q, 20p, segmental 20q, please note there were several separate regions of copy number gain on 7q and 20q, and part of the 7q gain was close to the threshold used to call as amplification.
