Bx proven eccrine porocarcinoma, Wide local excision, 2cm margin down to fascia.

Macroscopic description:
An orientated ellipse of skin 40x93x10mm. In the centre of the specimen there is a relatively well-defined cream to tan nodular lesion measuring 23x50mm with a height of approximately 10mm.

Asymptomatic solitary infiltrative erythematous swelling on the right neck 8 x 7mm of one month duration. ? malignancy.

Macroscopic description:
A 4 x 4mm skin punch to a depth of 5mm.

Immunohistochemistry:
The majority of cells are positive for CD3, CD5 and CD4 with focal down regulation of CD7. A few CD8 positive cells and CD20 positive cells are present. PD1 stains several of CD3/CD4 positive cells. No follicular dendritic cell meshwork noted with CD21 and CD23. Ki-67 index is 10%. PCR - Clonal T cell receptor beta chain gene rearrangement detected.
