Progressive shortness of breath. High resolution CT scan showed basal opacification with intervening ground glass opacities. No honeycomb change but some traction bronchiectasis.

Left upper lobectomy for incidental finding of tumour on CXR. DPAS positive (focal). TTF-1 positive. P63 negative.

CXR revealed mass left lower lobe. DPAS negative. Immuno showed positive staining for p63. Negative staining for TTF-1.

Right lower lobe mass hot on PET scan. Frozen section inconclusive. On immunochemistry the larger cells are positive for CD20 and also show weak focal staining with CD79a. They are negative for cytokeratins (CAM5.2 and MNF-116), ALK-1, CD30, CD15 and CD56. CD43 shows diffuse strong positivity. EBV in-situ hybridisation shows nuclear positivity in many small and large lymphoid cells (up to 30%).

Known metastatic papillary carcinoma of thyroid and rhuematoid arthritis. Further mass identified in right lower lobe. ANCA, ZN, grocott and DPAS negative.

Male 53 years

Crohn's disease. Shortness of breath diffuse parenchymal changes on CT. Currently being treated with steroids and infliximab.



Specimen: VATS biopsy

Female 40 years

Previous resection of synovial sarcoma. Mass right lower lobe of lung. Immunocytochemistry shows negative staining for cytokeratins, S100, SMA and desmin. There is strong positive staining for BCL2, and weak focal positive staining for EMA. Original tumour was of biphasic type.

Interstitial changes on CT. Smoker. No weight loss.

Previous resection for colorectal carcinoma, 6 months previously, now nodule discovered right upper lobe. TTF-1 positive, CK 20 negative.
