Case 1 - Birmingham

Female 19 years

Failed allograft (right lobe); Early graft dysfunction in both recipients of the same split liver, both requiring re-transplanting at day 8. It subsequently emerged that the donor died from thrombotic complications related to the Astra-Zeneca covid vaccine.

Case 6 - Edinburgh - first biopsy

Male 64 years

First biopsy late June 2021 - 'New lymphadenopathy, informal report of suspicion of lymphoma on LN biopsy [at peripheral hospital - referred to a second centre]. Previous toxoplasmosis.' Report of referred LN biopsy - post-transplant lymphoproliferative disorder (PTLD), described to show large blast-like B cells which are positive for CD30, MUM1 and EBERs in situ hybridisation. Immunophenotype on liver biopsy - scattered large B cells present in the liver biopsy, although MUM1 positive, do not express CD30 and are negative for EBERs (EBV) on in-situ hybridisation. Treated with methylprednisolone and rituximab for EBV-driven post-transplant lymphoproliferative . Disorder as first biopsy was being reported.

Case 6 - Edinburgh - second biopsy

Male 64 years

Second liver biopsy early August 2021 - 'Lymphadenopathy improved after treatment. LFTs worsened - Bilirubin 708, ALT 184, ALP 1860.'

Recurrent PSC on MRCP. Treated with increased dose of ciclosporin, methylprednisolone once off, and restarted on MMF.
