Small adenoma, increasing in size, risk of HCC transformation. No cirrhosis. No hepatitis.

Macroscopic description: Lobe of liver weighing 405 gms and measuring 190 x 130 x 70 mm. On slicing: spherical white lesion 20 mm in diameter close to resection margin.

Case number 293

Female 60 years

Patient complained of mild upper right quadrant discomfort. CT scan showed a few nodules ?HCC, resulting in a partial hepatectomy.

Specimen: Liver

Macroscopic description: Left hepatectomy weighing 354 gms and measuring 150 x 90 x 40 mm. The cut surface shows four well defined yellow/brown nodules (4 mm (?4 cms) in max dimension). Microscopically all nodules had similar features to that included

Cirrhosis suggested on CT scan, ascites and decreased albumin but coagulation normal. Metabolic/viral/genetic screens negative. No alcohol excess. Masson stain: severe fibrosis with many fibrous septa.

Case number 295

Female 76 years

CT showed large hepatic mass. No other tumour. Presumed primary hepatoma. (incidental ovarian fibroma at surgery - frozen section confirmed). Immunohistochemistry - positive staining with CK7, CK18, CK19, CD56. Negative staining with CK20, CDX2. CD10 and CD13 - no canalicular staining pattern alphafeto protein, non contributory (problems in lab with pos controls). Gall bladder removed - no tumour.

Specimen: Right hepatectomy

Macroscopic description: 18 x 18 x 12 cm segment of liver. Subcapsular 15 x 9 x 7 cm tumour, white. Extended close to cut margin.

PC Feb 2006. Nausea, anorexia, jaundice, pale stool and dark urine. ALT 468, alpha GT 1827, BR97, alk phos 517, ferritin 1660. Heterogenous liver on USS. Drugs - black cohosh. Alcohol 10-12 units/week. Stopped black cohosh April/May 06.

Liver biopsy Dec 2006. Hep A, B, C negative. AIP negative becoming positive ANA (1:80) in Feb 2007. LFTS now improving - no treatment given.

Cryptogenic cirrhosis.

Macroscopic description: Weight 1,054gms, measurement 23 x 13 x 6cms. Partial nodularity (1-6mm diameter nodules) alternating with areas of parenchymal collapse.
