Specimen:
Liver biopsy.
Macroscopic description: Liver core measuring 26mm.
Immunohistochemistry: H&E and EPSR.

Four week history of progressive jaundice. Has been taking "gain supplement" Anadrole from the internet.
Specimen: Liver core biopsy.
Macroscopic description: A green and yellow speckled core measuring 25mm in length.
Immunohistochemistry: Picrosirius red, CK7.
Case number 3
Female 36 years
Complex cyst S4B. Robotic excision of liver cyst. On retrieving cyst in a bag through the wound, the cyst ruptured in the bag.
Specimen: Segment 4b cyst excision.
Macroscopic description: Multiloculated cyst measuring up to 55mm, with surrounding thickened white area at interface with liver.
Immunohistochemistry: None.

Segment 6 lesion 1.6cm ?HCC.
Specimen: Liver lesional biopsy.
Macroscopic description: Four cores of tissue longest 25mm.
Immunohistochemistry: EPSR, LFABP, glutamine synthetase, serum amyloid A, B-catenin.

Background of UC. Diagnosed with small duct PSC in 2007 with biopsy.
Specimen: Liver core biopsy.
Macroscopic description: A cream tan core measuring 27 mm.
Immunohistochemistry: Victoria blue, CK7, Van Gieson.
Case number 6
Male 72 years
Known polycystic liver. Hard lesion left lateral segment. PET AVID ?malignant.
Specimen: Liver resection, left lateral.
Macroscopic description: Polycystic appearance to liver. On slicing pale white-yellow discoloured lesion (65mm diameter) associated with a number of the cysts.
Immunohistochemistry: None.

Signs of severe autoimmune hepatitis. ALT 1487, GGT 201, ALP 222, bilirubin 22.
Specimen: liver biopsy.
Macroscopic description: 30mm core bisected.
Immunohistochemistry: HVG. (negative copper/copper associated protein stains).

Female 56 years
Multiple liver lesions. Leiomyosarcoma of right thigh 2009. One liver lesion biopsied.
Specimen: Lesional liver biopsy - H&E.
Macroscopic description: five cream cores measuring 12, 14, 12, 11 and 5mm plus fragments.
Immunohistochemistry: Strong staining: actin, calponin and caldesmon. Very focal strong staining for MNF116. Negative: desmin and S100.

Male 64 years
Right renal mass with liver metastasis. Also lymph node hepatic artery. RCC plus liver met - resected separately, node then resected.
Specimen: Liver resection.
Macroscopic description: Segment 5 and 6 of liver with firm and white tumour, 25mm maximum size. Also received were right kidney with tumour, gallbladder and hepatic artery lymph node (no slides of these specimens provided).
Immunohistochemistry: ERG, CD31, CK7, CAMTA1.

Female 58 years
Autoimmune hepatitis diagnosed 2001.
Liver biopsy 2013 revealed chronic inflammatory infiltrate with mild interface activity but no significant progression.
Liver biopsy February 2019 inflammatory activity 5/18 fibrosis grade 1/6. Nil significant deterioration compared with previous biopsy. LFTs remain fluctuant despite Azathioprine 150 mg daily. Reassess disease activity-considering mycophenolate mofetil. Additional information from clinic letter August 2024: LFTs 01/08/24 bilirubin 9, ALT 98, alkaline-phosphatase 381.
Specimen: Liver core biopsy.
Macroscopic description: A single cream core received measuring 24 mm.
Immunohistochemistry: CK7, Victoria blue, Van gieson.

Ultrasound cirrhotic liver. Right heart failure. Being considered for heart transplant. ?cirrhosis/portal hypertension.
Specimen: liver biopsy.
Macroscopic description: 30mm core bisected.
Immunohistochemistry: HVG.

Liver lesion segment 2 ?AML ?adenoma. previous liposarcoma resected from left lower leg 3 years ago.
Specimen: Wedge resection, segment 2.
Macroscopic description: Wedge of liver containing yellow discoloured lesion which extends to the resection margin.
Immunohistochemistry: None.

Female 20 years
Open right hemihepatectomy for large cystic lesion of the right lobe (segment 8, 6, 7, 5).
Specimen: Liver resection.
Macroscopic description: Right hemihepatectomy with large tumour, 300 x 175 x 135mm. Tumour necrotic, multicystic, mucoid with solid white and cystic components. Cystic components filled with tan mucoid material.
Immunohistochemistry: DPAS, A1AT, Glypican3, AE1/AE3, CD31.
