T4 N0 Right maxillary tumour. Presented with tenderness right maxilla/cheek + numbness + one episode of epistaxis.

Specimen: Right hemi-maxillectomy with neck dissection.

Macroscopic description:
Right hemi-maxillectomy specimen measures 85 mm (bucco-palatal), 79 mm (antero-posterior), and up to 63 mm (supero-inferior). Two central coronal slices show tan, gelatinous tumour with cross section of 62 mm.

Immunohistochemistry:
Diffusely positive for vimentin. Patchy but focally strong expression of pan-cytokeratin AE1/AE3, CK7, and CK14. Negative: SMA, calponin, desmin, Myo-D1, STAT-6, myogenin, beta-catenin, CD34, S100, CK5, EMA, p63, GFAP.

POSITIVE: Pan-cytokeratin AE1/AE3, CK7, HBME-1, Pax-8, TTF-1, thyroglobulin, Cyclin-D1, CD56 (patchy). NEGATIVE: BCl-2, p53, CK19. Ki67: 10-30% proliferation fraction.
