Peritoneum. Faecal peritonitis from obstructing rectal carcinoma. Multiple nodules in peritoneum with perforated caecum.

Macro: A piece of omentum 105x75x25mm. Two nodules are identified in the omentum, these measure 13 and 17 mm each.

Specials: The tumour shows very weak positivity for CDX2, patchy positivity for CK20 and is negative for CK7.

Macro: Gallbladder received opened measuring 75 x 30 mm with a wall thickness of 3mm. On opening there are 2 polyps within the body and fundus measuring 4x4x4mm and 3x4x3mm. The background mucosa looks unremarkable. No gallstones present. Section of polyps taken.

Pancreas. Tail of pancreas lesion. Cytology from FNA of the lesion: NET.

Macro: Distal pancreatectomy showing a 45 mm, round, well-demarcated mass, partly solid and partly cystic.

Specials: Immunohistochemistry shows positivity for synaptophysin, CD56, CD10 and beta-catenin nuclear +) and negativity for CAM5.2 and chromogranin.

Oesophagus. Pedunculated mass obstructing upper oesophagus. Became detached from pedicle during surgery. Mass and oesophagus submitted separately.

Macro: A lobulated mass of tissue with a smooth capsule measuring 130 x 65 x 40mm. On cut section, the mass is soft with a white/cream cut surface.

Specials: Submitted section is from the mass. The other twelve sections taken all appeared similar. The tumour was S100 and MDM2 positive. There was no specific staining with CD117, DOG1, SMA, GFAP or NFP.

Large intestine. Emergency right hemicolectomy for perforated appendiceal tumour.

Macro: Large perforated tumour mass involving appendix, caecum and terminal ileum with macroscopically involved nodes. Proximal and distal resection margins appear normal. The tumour was a moderately differentiated adenocarcinoma of appendix, pT4N1. The submitted section is from the distal resection margin.

Specials: Tumour cells CD20+ve. CD5, CD10, bcl-6, cyclin D1 -ve. Ki67 <10%.

Macro: Right hemicolectomy 410mm in length including 25mm ileum. Bulky tumour 60mm across within fatty tissue adjacent to caecum. Multiple involved lymph nodes.

Specials: Tumour cells CK7 +ve, CK 20 -ve, CA125 +ve, WT1 +ve

Large intestine. Sigmoid colectomy for a distal sigmoid lesion ?malignant ?GIST.

Macro: Segment of large bowel 110mm in length. At the midpoint of the segment, the bowel wall is distorted by a firm pale ill-defined lesion, up to 28 mm in maximum dimension. The lesion appears to extend to the serosal surface. The overlying mucosal surface is intact but the lumen is compressed into a crescent shape in cross-section.
