Female 24 years

Abdominal mass, ?GIST ?sarcoma ?ovarian. CT scan: large solid mass lesion which may be arising from stomach or could be a peritoneal deposit. There is widespread peritoneal disease, bilateral adnexal masses and subcapsular liver deposits. Percutaneous biopsy of the large left sided lesion is suggested to determine.

Macro: multiple fragments in aggregate measuring 50 x 10 x 10mm (laparoscopic biopsies).

Specials: alpha 1 AT, CD10, vimentin, NSE strongly +ve. CD56 and synaptophysin dim +ve. Chromogranin, TTF1, CA19.9, Calretinin, cKit, CK7, CK20, BerEP4, CA125 and desmin, alphaFP, HCG, inhibin, SMA and AE1/AE3 all negative.

Macro: wedge excision specimen 30 x 25 x 12mm. It consists of mucosa, submucosa and muscularis propria. A circumscribed tan nodule 18mm maximum dimension is present in the muscularis propria.

Specials: smooth muscle actin positive, pan CK negative, S100, CEA, Desmin negative.

Macro: fibrofatty tissue measuring 160 x 10 x 50mm. No cyst seen.
