Oesophagus. Dysphagia. Trachealisation of the oesophagus, more pronounced in upper and mid oesophagus. Grade 1 oesophagitis at GOJ.

Colon: biopsies from appendiceal orifice. Known ulcerative colitis, mildly active. Symptoms settle completely with enemas and therefore stopped taking them and then symptoms returned. Rectum inflamed oedematous and bled on contact. The rest of the colon looked macroscopically normal until the appendix orifice which again was inflamed. TI looks normal. (NB histology of multiple biopsies from rest of colon and ileum entirely normal. Rectal biopsies show acute and chronic inflammation and crypt distortion only).

Gall bladder. Subtotal cholecystectomy.

Macro: A segment of gallbladder including body and fundus measuring 5.4 x 3.3cm. The serosa appears congested. There are no gallstones in the lumen or the pot. The mucosa appears focally congested. The wall has a maximum thickness of 1.2cm and has cystic areas at the fundus.

Osophagus. Known squamous HGD dysplasia at least on previous bx within the middle oesophagus. Stomach and duodenum - Normal. TGF 38cm. 2% Lugol's iodine staining with nodularity at 29 - 31cm and pink sign 28 to 33cm. Extensive hemicircumferential EMR (5 bands).

Macro: Four irregular pieces of roughened mucosa ranging from 0.7 x 0.5 x 0.4cm up to 1.2 x 0.9 x 0.4cm.

Colon. Altered bowel habits tending towards diarrhoea. Normal examination to terminal ileum. Right and left colon biopsies taken to exclude microscopic colitis (these are 'right colon' biopsies).
