Case BE1

Male 57 years

Mediastinal mass. EBUS and mediastinoscopy inconclusive.

Specimen: Left thoracotomy and open biopsy of mediastinal mass. Three irregular tan fragments ranging in size from 5 mm to 11 mm in maximum dimension.

Special stains/IHC results:
Positivity for CD56, chromogranin and synaptophysin. S100 stains dendritic cells. AE1/AE3, MNF116, melanin A, TTF-1 and HMB-45 are negative. Ki-67 proliferative index 2%-3%.

Male 64 years

IHD, rheumatism. Pleural effusion, nodular appearance to pleura ?malignancy, ?rheumatoid.

Specimen: Pleural biopsy - Three pieces of grey/yellow membranous tissue, ranging from 26 x 10 x 1 mm to 25 x 15 x 2 mm. The surfaces appear smooth and macroscopically unremarkable.

Special stains/IHC results:
Surface cells diffusely positive for CD68 with scattered nuclei positive for WT1.

Case BE3

Female 60 years

Right pleomorphic adenoma (parotid).

Specimen: Right superficial parotidectomy - A single piece of nodular, grey/brown tissue measuring 41 x 25 x 21 mm. Specimen is partially covered by a shiny membrane over an area of 33 mm, the remaining surface is ragged. Specimen weighs 8.3g. Slicing through the tissue reveals a discrete, yellow/white nodule measuring 10 x 10 x 7 mm.

Special stains/IHC results:
Lesion strongly positive with AE1/3, CK7 and p63. p40 shows patchy weak positivity. S100 shows patchy very weak positivity. Sox10 is negative in the tumour cells. FISH Result: MAML2 gene rearrangement detected.

Case BE4

Female 65 years

Right nephroureterectomy for ?TCC ureter.

Specimen: Nodular expansion of ureter over a distance of 40mm, up to a thickness of 15mm. On slicing, wall markedly thickened with collapsed lumen.

Special stains/IHC results:
CD10 is focally positive. GATA-3 is negative in these areas (but stains the urothelium of the ureter).

Case BE5

Male 35 years

Right inguinal orchidectomy, suspected tumour.

Specimen: An orchidectomy specimen weighing 30 g and comprising testis measuring 35 x 35 x 35 mm and spermatic cord measuring 30 mm in length and up to 25 mm in diameter. On slicing a circumscribed tumour measuring 28 x 20 x 25 mm is present almost replacing the entire parenchyma except a thin concentric rim of normal tissue on the peripheral aspect. It has a variegated appearance with dark and light yellow areas and a few areas of haemorrhage and necrosis.

Special stains/IHC results:
On immunohistochemistry, the larger tumour is strongly positive for Oct 3/4, CD30 and CAM5.2 with focal positivity for PLAP. Staining for CD117 is negative. The smaller tumour component is positive for Oct 3/4 and CD117 and PLAP (focal patchy). Staining is negative for CAM5.2 and CD30.

Right partial parotidectomy - Suspected right parotid pleomorphic adenoma.

Specimen: Fibrofatty tissue, measuring 56 x 27 x 32 mm, with an attached firm nodule measuring 30 x 23 x 22 mm. The nodule appears encapsulated.

Special stains/IHC results:
The atypical cells are positive for AR, GATA 3, GCDFP-15 but negative for S100 and DOG-1.

R hemithyroidectomy.

Specimen: R thyroid lobe 60 x 40 x 35 mm containing a well-defined, 40 mm nodule with homogenous, tan-coloured, solid cut surface. No cystic, haemorrhagic or necrotic areas. The nodule occupies the lower 2/3rd of the lobe. Rest of thyroid appeared normal.

Left lower lobe biopsy proven typical carcinoid.

Specimen: A lobe of lung containing a peripheral cream coloured lesion measuring 31 x 25 x 40 mm.

Special stains/IHC results:
Tumour positive for CD56 and synaptophysin, variably for TTF-1, negative for chromogranin, p40 and napsin. Ki67 proliferation factor is approximately 50%.

Male 69 years

Left lobe of thyroid. Hypoechoic lesion. Radiology U4. FNA Thy3f.

Specimen: A left hemithyroidectomy specimen measuring 70 x 36 x 25 mm. The lower and middle pole is occupied by a solitary, circumscribed and fairly demarcated thinly rimmed pale yellow nodule measuring 32 x 28 x 20 mm. The cut surface of the nodule is pale yellow with areas of haemorrhage and fibrosis. The nodule almost abuts the capsule.

Special stains/IHC results:
IHC - negative for Calcitonin. Focal vascular invasion and capsular disruption noted in another section.

Female 78 years

Excision lesion L axilla.

Specimen: Skin ellipse 60 x 50 x 10 mm. Slicing revealed a myxoid cut surface involving the full thickness of the skin and the subcutaneous fat.

Special stains/IHC results:
Focal positivity for SMA and CD34. No staining for desmin, h-caldesmon, S100, AE1/AE3, p16 and AR, with retention of Rb1 expression.
