Clinical details: Cutaneous metastasis. Renal cancer. Type of specimen & macro examination: Excision right temporal scalp. Unorientated ellipse of skin measuring 40 x 21 x 5mm. On the surface, there is a polypoid nodular mass measuring 20 x 18 with a height of 15mm and measures 2mm away from the closest peripheral margin. The deep margin is inked green. The cut surface reveals a pale nodule with cystic areas and foci of haemorrhage. Previous: Left iliac bone lesion, biopsy; metastatic renal cell carcinoma.

Clinical details: Epidermal cyst. Type of specimen & macro examination: Excision right forearm. An unorientated ellipse of skin measuring 20 x 12 x up to 10mm. The surface shows a pale ill-defined nodule measuring approximately 10 x 8 x up to 3mm. Cut surface reveals a well defined pale nodule just below the skin and extending to the sub-cutaneous tissue and measures approximately 7 x 6 x up to 7mm.

Clinical details: Lesion left shoulder. Poorly diff. SCC (clinically). Resection 1cm margin, deep to fascia. Marker stitch 12 o’clock. Type of specimen & macro examination: Skin 50 x 47 x 15mm with a suture attached to its 12 o’clock margin. There is an exophytic grey nodule 25 x 17mm and 4mm in height.

Case BD8 Category A (scoring)

Female 76 years

Clinical details: Perforated sigmoid with generalised peritonitis - sigmoidectomy. Hartmann's procedure. Type of specimen & macro examination: 'Sigmoid' – A length of sigmoid colon measuring 155mm long. 15mm from the end resection margin, there is a full-thickness defect measuring 12mm across. The serosal surface of the specimen shows a purulent exudate. The mucosal surfaces appear normal away from the first defect. Towards the mid-specimen, there is an area of bowel wall thickening (up to 20mm in thickness) with a second possible wall defect. There is an irregular nodular area adjacent to the open end margin measuring 38mm across at towards the outer specimen surface. A further pale nodule 28mm across is seen towards the other end margin. No lumenal tumour is seen. There are no diverticula.

Special stains/IHC results:
Lesional cells - positive; CK7, OR, PAX8, WT1, p53 (diffuse and strong). Lesional cells - negative: CK20, CDX2.

Clinical details: Large fibroid uterus. Emergency hysterectomy done due to bleeding. Type of specimen & macro examination: Uterus with cervix, both tubes and ovaries. Uterus 245mm fundus to os x 195mm transverse x 65mm antero-posteriorly, 3674g. Ectocervix and serosa normal. Endometrium 1mm in thickness. The myometrium includes a single well circumscribed mass measuring 215 x 135 x 135mm. The centre of the mass is pale and necrotic. Attached tubes and ovaries normal. Section of mass.

Clinical details: Previous treatment for lymphoma. Left tonsillar enlargement. Left tonsillectomy. LIMS: previous diagnosis mantle cell lymphoma level V neck node (6 years previously). Type of specimen & macro examination: Two pieces of tonsil, 30 x 18 x 20mm and 20 x 15 x 8mm.

Clinical details: Tender large inguinal node. HIV positive patient. Electronic patient record: clinicians sent lymph node for PCR testing off site - positive for Chlamydia trachomatis. Type of specimen & macro examination: A piece of lymph node tissue measuring 30 x 25 x 15mm.

CD68+ groups of macrophages, CD3 & CD20 highlight a zonal distribution of T and B cells, CD138+ plasma cells polytypic with kappa and lamba, CD15+ neutrophils, AE1/AE3 negative, syphilis IHC shows focal probable contaminants.

Neoplastic cells - positive: vimentin, CD34, BCL2, beta catenin (cytoplasmic), STAT6 (nuclear). Negative: cytokeratin cocktails (AE1/AE3, MNF116), CD31, EMA, smooth muscle actin, smooth muscle myosin, desmin, S100, calretinin, thrombomodulin, DOG1 and HMB45. MIB-1 shows a low proliferation rate (approximately 5%).
