Clinical details: Cutaneous metastasis. Renal cancer. Type of specimen & macro examination: Excision right temporal scalp. Unorientated ellipse of skin measuring 40 x 21 x 5mm. On the surface, there is a polypoid nodular mass measuring 20 x 18 with a height of 15mm and measures 2mm away from the closest peripheral margin. The deep margin is inked green. The cut surface reveals a pale nodule with cystic areas and foci of haemorrhage. Previous: Left iliac bone lesion, biopsy; metastatic renal cell carcinoma.

Clinical details: Epidermal cyst. Type of specimen & macro examination: Excision right forearm. An unorientated ellipse of skin measuring 20 x 12 x up to 10mm. The surface shows a pale ill-defined nodule measuring approximately 10 x 8 x up to 3mm. Cut surface reveals a well defined pale nodule just below the skin and extending to the sub-cutaneous tissue and measures approximately 7 x 6 x up to 7mm.

Clinical details: Lesion left shoulder. Poorly diff. SCC (clinically). Resection 1cm margin, deep to fascia. Marker stitch 12 o’clock. Type of specimen & macro examination: Skin 50 x 47 x 15mm with a suture attached to its 12 o’clock margin. There is an exophytic grey nodule 25 x 17mm and 4mm in height.

Clinical details: Lesion L epididymis. Type of specimen & macro examination: A nodular, but smooth surfaced, irregular piece of grey white tissue 11 x 11 x 10mm. It has a uniform grey white solid cut surface. Special stains/IHC results None.

Clinical details: One of several nodules on scalp. Type of specimen & macro examination: Received is an ellipse of skin 11 x 7 x 8mm. The cut surface reveals an ill-defined lesion immediately beneath the skin surface at least 5mm across reaching one peripheral resection margin.

Clinical details: Right nasal polyp. Right nasal obstruction. Polyp in nasal cavity medial to middle turbinate and right maxillary sinus. ? neoplastic. Type of specimen & macro examination: 'Right nasal polyp'. Multiple fragments of friable brown soft tissue ranging from 5mm up to 13mm in maximum dimension. Representative section.

Special stains/IHC results:
Positive: CDX2, CK20. Negative: TTF1, CK7.

Female 76 years

Clinical details: Perforated sigmoid with generalised peritonitis - sigmoidectomy. Hartmann's procedure. Type of specimen & macro examination: 'Sigmoid' – A length of sigmoid colon measuring 155mm long. 15mm from the end resection margin, there is a full-thickness defect measuring 12mm across. The serosal surface of the specimen shows a purulent exudate. The mucosal surfaces appear normal away from the first defect. Towards the mid-specimen, there is an area of bowel wall thickening (up to 20mm in thickness) with a second possible wall defect. There is an irregular nodular area adjacent to the open end margin measuring 38mm across at towards the outer specimen surface. A further pale nodule 28mm across is seen towards the other end margin. No lumenal tumour is seen. There are no diverticula.

Special stains/IHC results:
Lesional cells - positive; CK7, OR, PAX8, WT1, p53 (diffuse and strong). Lesional cells - negative: CK20, CDX2.

Clinical details: Rectal biopsy. No other clinical details provided by clinician. Type of specimen & macro examination: Three pieces of tissue measuring 9-15mm across. The cut surface of the larger piece is pale and homogenous.

Lesional cells - positive: synaptophysin, CD56, chromogranin (patchy), CDX2 (patchy). Lesional cells - negative: CK7, CK20. Lesional cells - Ki67 index >80%.

Clinical details: Large fibroid uterus. Emergency hysterectomy done due to bleeding. Type of specimen & macro examination: Uterus with cervix, both tubes and ovaries. Uterus 245mm fundus to os x 195mm transverse x 65mm antero-posteriorly, 3674g. Ectocervix and serosa normal. Endometrium 1mm in thickness. The myometrium includes a single well circumscribed mass measuring 215 x 135 x 135mm. The centre of the mass is pale and necrotic. Attached tubes and ovaries normal. Section of mass.

Tumour cells - positive: desmin, smooth muscle actin.

Clinical details: Previous treatment for lymphoma. Left tonsillar enlargement. Left tonsillectomy. LIMS: previous diagnosis mantle cell lymphoma level V neck node (6 years previously). Type of specimen & macro examination: Two pieces of tonsil, 30 x 18 x 20mm and 20 x 15 x 8mm.

Special stains/IHC results:
Positive: CD20 (nodular/follicular areas), BCL2 (except centre of follicles/germinal centres), cyclin D1 (nodules/periphery of follicles), CD3 (cells mainly in inter-follicular areas; some cells in nodules/follicles), CD10 (centre of follicles/germinal centres).

Clinical details: Tender large inguinal node. HIV positive patient. Electronic patient record: clinicians sent lymph node for PCR testing off site - positive for Chlamydia trachomatis. Type of specimen & macro examination: A piece of lymph node tissue measuring 30 x 25 x 15mm.

Special stains/IHC results:
CD68+ groups of macrophages, CD3 & CD20 highlight a zonal distribution of T and B cells, CD138+ plasma cells polytypic with kappa and lamba, CD15+ neutrophils, AE1/AE3 negative, syphilis IHC shows focal probable contaminants.

Clinical details: Right submandibular lump (possibly lymph node). Type of specimen & macro examination: Right submandibular lymph node. Smooth, slightly lobulated, fatty mass measuring 55 x 37 x 52mm. On slicing, it has a soft, slightly rubbery, pale, tan, homogenous cut surface.

Special stains/IHC results:
Positive: CD20, CD79, IRF4, OCT-2, PAX-5. Negative: CD15, CD30, LMP-1, IgD.

Clinical details: Lump R groin above inguinal ligament. ?Lymph node. Type of specimen & macro examination: Received is a grey nodule with attached fatty tissue. 50 x 30 x 20mm. The external surface is general smooth. The cut surface is grey and homogenous.

Special stains/IHC results:
Neoplastic cells - positive: vimentin, CD34, BCL2, beta catenin (cytoplasmic), STAT6 (nuclear). Negative: cytokeratin cocktails (AE1/AE3, MNF116), CD31, EMA, smooth muscle actin, smooth muscle myosin, desmin, S100, calretinin, thrombomodulin, DOG1 and HMB45. MIB-1 shows a low proliferation rate (approximately 5%).

Clinical details: Solid cystic lesion left breast. Type of specimen & macro examination: Left breast wide local excision. Fibrofatty tissue weighing 17g orientated with sutures. A needle wire is present. Measures 42 x 38 x 25mm. The specimen is sliced into 6 slices. Within slices 1 and 2 there is a cystic lesion 12 x 8 x 15mm. Section taken from slice 2.

Special stains/IHC results:
Positive: CK14, CK5/6, p63, S100, CD10, SMA (patchy/focal). ER 2%. Negative: CD34, desmin, PR, HER2.
