Case BC1 Category A (scoring)

Female 62 years

Clinical details: Paget's disease right breast. Right central excision, short superior, long lateral. Type of specimen and macro examination: Wide local excision specimen comprising of an ellipse of skin with nipple and underlying breast tissue. The ellipse of skin measures 60 x 25mm. the nipple is 15mm in diameter. Underlying breast tissue is 65 x 35 x 25mm. The specimen is orientated by the attached long suture marking the lateral margin, the short suture at the superior margin. The whole specimen weighs 29g. The posterior margin of the specimen is painted black, superior margin painted red and inferior margin is painted green. The anterior aspect is covered by the skin. On slicing there is an ill-defined firm whitish area of fibrosis which measures 14 x 25 x 14mm. The lesion is located 8mm from the posterior margin, 20mm from the lateral margin, 4mm from the inferior margin and 15mm from the medial margin. The fibrosis appears to extend to the superior margin. The anterior aspect is covered by the nipple and the nipple is showing an area of ulceration up to 10mm in diameter.

Case BC2 Category A (scoring)

Female 36 years

Clinical details: 2 x 2 left vulva lesion, painful left vulva region. Type of specimen and macro examination: An irregular piece of skin which measures 26 x 25 x 7mm. The surface appears nodular and roughened. No orientation was provided and the specimen has been bread sliced.

p16 is negative.

Female 27 years

Clinical details: 26 x 18 x 20mm. Rt thyroid solid lesion containing microcalcification. Type of specimen and macro examination: Total thyroidectomy. This measures 70 x 45 x 20mm. On slicing there is a pale solid nodular lesion and this measures 29mm SI, 20mm AP and approximately 15mm medial to lateral. This appears restricted to the right lobe and does not extend into the isthmus.

Lesional cells positive for CK7, GATA-3 and ER. They are negative for CK20.

Male 81 years

Clinical details: Lesion SCC posterior scalp (marker stitch 12 o'clock) excised with 10mm margins. Previous h/o excision SCC lesion from scalp. Squamous cell carcinoma ?? Type of specimen and macro examination: An oval piece of skin measuring 42 x 38 x 4mm with a suture at one edge which marks 12 o'clock. The surface bears a slightly irregular but well circumscribed nodular lesion measuring 19 x 15 x 5mm. Towards the 8 o'clock margin the surface appears rough over an area of 9 x 9mm.

Special stains/IHC results:
The lesional cells are negative for AE 1/3, MNF116, 34BE12, CK14, CK5/6, p40, p63, S-100, melan-A, CD34 and desmin. They show patchy weak positivity for SMA and diffuse positivity for CD10. There is focal weak positivity for ERG. CD31 stains background inflammatory cells, but the lesional cells are negative.

Clinical details: Anterior mediastinal tumour invading left phrenic N. Type of specimen and macro examination: A firm irregular white mass measuring 70 x 45 x 40mm.

Special stains/IHC results:
Tumour cells are positive for CD21, CD35, CD99, CD68, BCL2. Cytokeratins (AE1/AE3), EMA and p63 show patchy weak expression. Melan A, HMB45, ALK1, desmin, SMA, S100, CD34, other cytokeratins (MNF116 and Cam5.2) are negative.

Clinical details: The top of cervix on the right side was quite inflamed and friable and bits of cervix broke off into the vagina. ? Ca cervix. Cervical biopsy done along with a small cervical loop (which was normal on histology). Section is from the cervical biopsy.
