Female 78 years

Lung mass history of anterior resection for colorectal carcinoma. Lobectomy, white nodule 21mm in diameter at 35mm from the bronchial resection margin. Special stains: positive for synaptophysin and CD56 with equivocal staining for TTF-1. CK7, CK20, CDX-2 and chromogranin are negative. The Ki67 proliferation index is up to 12%.

Male 80 years

?mesothelioma. No definite asbestos exposure, CT suspicious of pleural disease. A VATS - some pleural thickening but not obvious malignancy. Multiple pieces of parietal pleura, the largest strip 40mm long. No focal lesion is seen. Special stains: Epithelioid cells are positive for AE1/AE3, calretinin and WT1 with weak positivity for CK5/6 but negativity for CEA and BerEP4.

Female 24 years

Tuberous sclerosis. Bilateral recurrent pneumothoraces. Multiple blebs on the surface of the lung. Surface bleb, 8mm in diameter. On sectioning there are multiple thin walled cystic cavities up to 3mm in diameter. Special stains: Spindle cells are positive for HMB45, oestrogen receptor, SMA (strongly) and b-catenin (weakly) but negative for Melan-A, S100, TTF-1 and CAM5.2. Papillary cells are positive for CAM5.2 and TTF-1 but negative for the other markers listed.

Female 50 years

Multinodular retrosternal goitre - main dominant L lower lobe, hanging independent of lower lobe. Total thyroidectomy. Left and right lobes appear normal. Hanging from the left lower lobe on a thin fibrous band is a 51mm nodular with a yellow and haemorrhagic cut surface. Special stains: Spindle cells positive for S100, CD34 and vimentin with patchy HMB-45. Negative for pan-cytokeratins, desmin, SMA, TTF-1 and EMA.

Female 28 years

'Stage I cervical carcinoma' treated abroad by excision only 2014. No radiotherapy and no clinical follow up. No other history available. Punch biopsy concerning; this section is taken from follow up LLETZ. Punch biopsy IHC: Ki67 mildly raised focally, strong nuclear positivity HNF1B but ER negative in worrying glands; HNF1B negative and ER positive in background endocervical glands.
