Male 66 years

Biopsy peritoneal nodules. Peritoneal malignancy. History of asbestos exposure.

Macroscopic description:
Multiple pale and haemorrhagic pieces 12mm in aggregate.

Immunohistochemistry:
Positive for CK7, CK5/6, Thrombomodulin calretinin, EMA and WT1. Negative for CK20, CDX2, CA19.9, PSA, TTF1, BerEp4, Desmin and CEA

Case J840

Male 56 years

Right testis. Orchiectomy. Tumour markers normal.

Macroscopic description:
An orchiectomy specimen measuring 70x50x35mm. The spermatic cord measures 30mm. On sectioning the testis measures 40x35x35mm and contains a well circumscribed, encapulated, yellow tumour which measures 23x18x16mm. The tumour is confined to the testis and does not reach the tunica albuginea. The background testicular parenchyma appears normal.

Immunohistochemistry:
the neoplastic cells are positive for inhibin, calretinin, Melan-A and vimentin. S100, HMB45, pan-cytokeratin, PLAP, CD30 and AFP are negative. HCG is equivocal. p53 stains scanty cells and the Ki-67 proliferation index is <1%

Female 27 years

Lung. Recurrent spontaneous pneumothorax. CT shows bilateral lung cysts. Pt is in the 2nd trimester of pregnancy and recently had an emergency nephrectomy.

Macroscopic description:
Clipped wedge of lung 45mm max containing tiny nodules and microcysts.

Immunohistochemistry:
spindle cells are positive for SMA, DES, HMB45, PR, weakly +ve for ER
