Presented with hypothalamic disturbance. MRI shows a cystic/solid/calcified suprasellar/hypothalamic lesion.

Presented via ophthalmologist with diplopia - VI cranial nerve palsy.PMHx - Type II diabetes mellitus. Radiologically large skull base lesion involving clivus, right petrous apex, sella, right jugular tubercle, both occipital condyles, with encasement of right common carotid artery, bulging into sphenoid sinus and involving right cavernous sinus with perineural spread through foramen rotundum. Specimen is incisional biopsies of nasopharynx and right sphenoid

Left parapharyngeal mass involving skull base removed with comprehensive neck dissection. Past history - Failure of response to chemoradiotherapy for p16 positive metastatic carcinoma in neck nodes. Recent biopsies showed low grade adenocarcinoma (retropharyngeal).

18 month history of anosmia. 1 year history of taste loss. No relevant past medical history. Family history of death at young age from breast / gynaecological malignancies.
